CERTIFICATION CHECK LIST.

The list for family visa or Tourist visa sponsor for Art. 15. Purpose

We just need to do certification and client will send it New Delhi, Portuguese Embassy to get visa.

For Dependent Worker

1. New Passport all pages, Old Passport only used pagers.
2. Resident card, if lost need lost document from Police, if under renewing need proof of renew. If Expired need that card and Appointment date.
3. If you have Last 3 year IRS + Liquidity.
4. 6 Months Bank Balance.
5. Work contract+ Declaration it is valid 15 day + ACT+ last 3 months pay slip
6. Junta or House contract + 3 last rent slip.
7. P.O.A by both Parent or single parents to Agent or single parent, in case of child.
8. Terms of responsibility, Family form, per person one, we do
9. Visa sponsor Letter, we need to make in office.

For Independent / Business worker

1. New Passport all pages, Old Passport only used pagers.
2. Resident card, if lost need lost document from Police, if under renewing need proof of renew. If Expired need that card and Appointment date
3. If you have Last 3 year IRS + Liquidity.
4. 6 Months Bank Balance.
5. If you have work contract+ Declaration + ACT+ last 3 months pay slip
6. Junta or House contract + 3 last rent slip
7. No Debt from Social and Finance from your company and also from you, in case of LDA
8. No Debt from Social and Finance from your personal activity
9. Last 6 months Business or activity balance Sheet
10. Proof of Activity and Permanent Certificate, in case of Lda.
11. POA by both Parent or single parents to Agent or single parent, in case of
12. Last 3 month IVA payment.
13. Terms of responsibility, Family form, per person one, we do
14. Visa sponsor Letter, we need to make in office.
