Checklist from Portugal for family member of Dependent worker *(who is working for someone)

For Wife / Husband –

1. Original Passport, 2. Criminal record from his country, 3 . Marriage registration from Portugal or home country,

4. Ticket and Boarding pass, 5. finance If he has social, If he has SNS Number.

For child –

1. Original Passport, 2. Birth Registration original copy, 3. PCC from his country if s/he is 16 year,

4. If he has relationship certificate from his country, 5. Agreement letter from the other parent if he is minor – less than 18 years. 6. University or School declaration if he is more than 18 years old, 7. Finance, 8. Ticket and  Boarding pass, 9. If he has social.

For parents-

1. Original Passport, 2. Criminal record from his country, 3. If you have Relation certificate, 4. Applicant / Your Birth registration.5. Finance 6. Social if he has, 7. Ticket and  Boarding pass.

Documents from home country must be dully legalized by MOFA from your Country and it is valid for 6 months from the date of issue.

Checklist from Portugal for main applicant, who is working for someone.

  1. Original Passport and Resident card, if card was lost need Polish record.
  2. Work contract, A.C.T, work declaration it is 15 days valid and Last 3 months Salary receipt.
  3. House contract or junta and agreement document with landlord or tenant it is 3 months valid.
  4. Last year I.R.S and declaration letter of liquidity.
  5. Last 6 months Bank Balance.

Checklist from Portugal for family member of Independent/ Manager/ Shareholder.

For Wife / Husband –

Checklist from Portugal for main applicant, who is working as Independent/ Manager/ Shareholder.

  1. Original Passport and Resident copy, if card was lost, need Polish record.
  2. Address Proof or House contract, if card address in not same in card.
  3. No debt Certificate from social or negative certificate,  from you and from your Lda, it is valid for 3 months.
  4. No debt Finance or negative  certificate, from you and from your lda, it is valid for 3 months.
  5. Opening of activity lda or Activity
  6. Last 3 months IVA Lda or Activity
  7. Last year IRC  Declaration from Lda, if lda is more than 1 year
  8. Last 6 months Balance sheet duly sign, stamp and sticker from Accountant
  9. Personal IRS and Liquidity of Last year
  10. Permanent  Certificate (Certidao Pernmente), if you have a Lda.
